Add/Remove/Change Retirement Beneficiary

Naming a beneficiary is an important part of managing your retirement account! Here are some tips you'll need to know if you need to name or change a beneficiary:

If you chose a PEBA-administered retirement plan (i.e., SCRS), you can make changes through Member Access.

If you do not use Member Access, you must submit an Active Member Beneficiary Form (Form 1102), which requires original signature(s) and notarization. Copies, altered forms, or forms with improper notarization will not be accepted or process by PEBA. *Note: Do not send a duplicate form to PEBA if you completed beneficiary changes in Member Access.

Members who participate in the State Optional Retirement Program (State ORP) must designate and update their State ORP account beneficiaries with their chosen service provider. Likewise, Deferred Comp participants must designate and update their beneficiaries with Empower Retirement.
