Download the Teacher Materials Order Form 25-26 and attach/submit completed form to Operations Administrator in an email.
The file is ready only, so once you open the link, go to File, Save As, Download a Copy to save it to your laptop for completion. Be sure to click "enable editing" so that your course will pop up on Column M and N in the "School Order Form" tab. 

Make sure that you use the teacher first and last name as listed in k12 PS.  You may leave Identity ID blank if you do not have it. 

The first few highlighted lines are just examples, do not include information there.
