If a Legal Guardian has requested to withdraw or transfer a student, below is a letter template the Student Support Teams send to the Legal Guardian.

Hello,

I am sorry to find out that you plan to withdraw your student from HVA.  In order to complete the withdrawal process we will need a records request or enrollment verification from the new school as well as the completion of this form. 

  1. Please complete the following parent withdrawal form:  https://na2.docusign.net/Member/PowerFormSigning.aspx?PowerFormId=b480b838-482c-4031-96af-ab42f19d0c8f&env=na2&acct=19cf31b0-043a-49f2-878e-6b8e17540f09&v=2  

  1. You can provide the following information to the new school to request your student’s records.

Fax: 864-558-0535

      3. Please also reply to this email with the name of the school that your student will be attending and your last date with Cyber Academy. 

      4. If the next school is a homeschool, please email the Membership Card or Welcome Letter that you received to records@heronacademysc.org.

Lamento saber que planea retirar a su hijo/a de HVA. Para completar el proceso de retiro, necesitaremos una solicitud de expedientes o una constancia de inscripción de la nueva escuela, además de que complete este formulario.

  1. Puedes proporcionar la siguiente información a la nueva escuela para solicitar el expediente de tu estudiante.
